Current  Open Positions

RN: Nurses function as part of the interdisciplinary care management team and participate in service planning for home care clients. Responsibilities include doing home-visit clinical nursing assessments of clients. Requirements: Must have a valid Registered Nurse’s license from the Commonwealth of Massachusetts and driver license or demonstrated ability to travel to designated facilities or consumer’s residences in a timely manner.  Knowledge of the home care system and community resources. Ability to communicate effectively and work well with other professionals. Computer skills are required. Incumbent must be able to carry a 15 to 20 lb. load to consumers’ homes 

Care Manager: Care Managers coordinate client services, reassess periodically and provide short-term crises management support to clients while performing responsibilities as part of a Home Care team. They maintain an ongoing case load of clients developing care plans, coordinating services, authorizing services and documenting actions according to standards.  Requirements: Bachelor degree in human services, social work, nursing or related field.  Multilingual people are encouraged to apply. Experience working with the elderly, strong interpersonal, organizational and documentation skills are necessary as are intermediate computer skills.
