Charaka Home Care Services

Our Services begin by reviewing your needs through discussions with involved family members; when necessary we consult with the physician(s) of the care recipient, social workers, hospitals, or nursing home staff. In working with an individual’s family, we develop an individualized program to manage your loved ones daily needs. We match their needs to the best possible caregiver, bring them to your home and ask you to participate in the selection process.

After the Charaka companion has been placed in your home, we continue our unique system of continued personalized contacts through telephone check-in, home visits. Most important, we continually communicate with the caregiver who is providing care to your loved one.

Our goal as a home health provider is to provide for the needs of the patient allowing the patient to remain living at home, regardless of age or disability. Sometimes, after surgery, a patient may require home care services that may range from such homemaking services as cooking or cleaning to skilled medical care. Some patients require home health aides or personal care attendants to help them with activities of daily living (ADL).

Home care is a form of health care service provided to a patient at home. Patients can receive home care services whether they live in their own homes, with or without family members, or in an assisted living facility. The reason for home care is to promote, maintain, or restore a patient's health and reduce the effects of disease or disability.
